On July 30, at approximately 6:05 p.m., the Story County Communications Center received a request for medical assistance at the Heartland Cooperative, located at 1120 West First Ave., Slater.

Segoviano Israel Rodriguez, 23, of Storm Lake, was working at the construction site of a new grain storage bin in Slater when the roof of the bin collapsed, pinning Rodriguez with debris. A crane was used to lift the roof structure, allowing Slater and Huxley Fire and Rescue crews to safely remove Rodriguez from the collapsed structure. Mercy One air ambulance transported Rodriguez with possible neck and back injuries to Mercy Medical Center in Des Moines.

The contractor building the grain storage bin is Manzano Grain Bin Services out of Waucoma, Iowa. The accident remains under investigation.
